Community Nursing for Children

The Children’s Community Nursing Team are experienced children’s nurses who provide specialist nursing care to children in their own home.  The team visit children who are referred to them, which may be for one or several visits.

 

Children with long term chronic conditions reduce the number of hospital visits/admissions by being supported by the Children’s Community Nursing Team, and they are able to provide training support for families/carers to enable children to be cared for in their own homes.

What area does the Home Care Team cover?

 

The Children’s Community Nursing Team visit children and families living or residing in the following areas:

 

Ashford, Sunbury, StanwellStanwell Moor, Walton, Staines, Charlton Village, Laleham, Weybridge, HalifordHersham,SheppertonEgham, Thorpe, BedfontEnglefield Green, Virginia Water, Feltham*, Hanworth*.

 

*if the child attends St. Peter’s Hospital.

 

 

What services does the team provide?

 

Children must have a specific need, for example:

 

·          Nasogastric tube/Gastrostomy tube feeding.

·          Intravenous medication.

·          Care of central venous access devices (HICKMAN® catheters and portacaths).

·          Pressure area care.

·          Care of Tracheostomy.

·          Palliative care.

·          Wound care management.

·          Oxygen therapy.

·          Overnight saturation monitoring.

 

 

How to access the service (Referral process)

 

The Children’s Community Nursing Team receives referrals from:

 

·          Ash and Oak Wards St. Peter’s Hospital.

·          Children’s Outpatient Unit.

·          Paediatric A&E.

·          Consultant Paediatricians.

·          Nursing Staff.

·          Medical Teams.

·          Health visitors.

·          Social workers.

·          Other hospitals including: The Royal Marsden, Great Ormond Street, West Middlesex, Royal Surrey County hospital.

 

The team also receives self-referrals from parents/carers. 

 

 

The team requests that written referrals are made so that details can be recorded and retained in the child’s notes for future reference, although referrals can be received by telephone.
