Learning Log

The RCGP is putting alot more emphasis on the quality and quantity of learning log entries. As such the Deanery,in conjunction with the RCGP, has brought out new specific guidelines for learning logs entries and quantity, this also includes new MANDATORY evidence. Please find details of these important changes below, in addition to a guide on reflective writing.
End of Year ARCP Deanery Review

The end of this academic year is fast approaching and all your eportfolios will be assessed by the Deanery review panel in June,in order to decide whether you have demonstrated enough development to progress to the next year or training. For ST3s this review will determine whether you can apply for you Certificate of Completion of Training and become an independent practicing GP in the UK. Below are the 2011 assessments timelines to help you.
For Attention of ST1s and 2s:

When you start in a new hospital job every four months the new Educational Agreement needs to be brought to the initial appraisal, read by both GP trainee and clinical supervisor and then signed:
  • New Educational Agreement

At your next initial appriasial and at subsquent initial appraisals, please ensure that you discuss the new educational agreement with your Clinical Supervisor. You will have to bring a copy of this to this meeting and discuss at interim and final appraisals. Both yourself and your CS should sign to say you have read and understand and also will action any agreed actions and timetables.

  • Being a Family Doctor

In addition to the educational agreement trainees should bring a copy of Being a Family Doctor to their initial appriasal and give it their Clinical Supervisor to read. The trainee should be familair with the contents of this document as it encapsulates the GP Identity and GP specific educational goals. This then means that you are well placed to discuss how to achieve these with your clinical Supervisor, within your 4 months.

Forms for starting in General Practice as either ST3 or ITP

When you start in practice you need two important forms filled in:
  • FPGPR1 form for your pay thorough training practice (signed by Educational Supervisor)

  • Medical performers list (see details below). All local practices are in Surrey PCT apart from Runnymede Practice, which comes under Berkshire.

ST3 Deanery Induction
Team Biographies
The Medical Perfomers List

If you are going to be working in General Practice (starting at ST3 or a GP training attachment in ST1 or 2) you must be on the Medical Performers List in order to take clinical responsibility seeing patients on your own.

If you are starting in a GP post you will need enhanced CRB (Criminal Records Bureau) clearance.

This section is important and should be read carefully. It is not possible to train in general practice unless the GPStR is entered on this list.

In order for a doctor to work in general practice, the Primary Care Trust (PCT), or its Agency, has a requirement to ensure that all appropriate checks are made and, for this, all doctors working in Primary Care will need to make an enhanced disclosure.

The Disclosure service enables organisations to make more thorough recruitment checks, particularly for positions that involve regular contact with children and vulnerable adults, via the Criminal Records Bureau (CRB).

Further details will be obtained on www.crb.org.uk.

This process is handled by the Primary Care Support Agency, or Shared Services Agency that covers the geographical area in which the Practice and PCT is situated. There are a number of agencies covering the KSS Deanery, and both the Hospital Trust and the General Practice will know these.

Although the exact process varies from agency to agency, all doctors will need to fill in a Disclosure Application Form and provide the necessary documents. There is good guidance to this on the CRB website. You must have received clearance from the CRB before you start seeing patients.

In many cases the doctors will need to present these documents personally to the primary care agency as copies may not be eligible. Doctors will also need to provide two clinical referees. Many doctors will have had such a check as part of the processes of employment carried out at the hospital, but the validity of this may expire with time, and all doctors hoping to take up a place in general practice should ensure that they have done this process through the relevant Primary Care Agency.

Please Note: This process can take up to 8 weeks and should be started as soon as possible. You should make contact with your intended training practice before you are due to start with them to allow time and ensure that they are aware and involved in this process.
