Completing NHS indemnity forms for equipment instruments on loan

Controls Assurance Standard - Medical Devices Management - Criterion 4

"Where devices are on loan (e.g. borrowed from a manufacturer or another hospital) it is important to be clear about where responsibility lies for any problems which may arise. Borrowed devices must go through an acceptance procedure. The central purchasing department should handle administration of the loan. The purchasing department should ensure indemnity agreements are signed for rented, loaned or borrowed equipment including equipment on trial"


Procedure

Please allow enough time to complete this process.

  • Stage 1 - Equipment identified

     
  • Stage 2 - Complete NHS Delivery Note for each item

     
  • Stage 3 - Form completed by both Supplier and Trust representatives

     
  • Stage 4 - Safety Check

     
  • Stage 5A - Items requiring Decontamination by Sterile Services

     
  • Stage 5B - Items not requiring Decontamination by Sterile Services
